Insurance:
Our staff will verify your benefits before your first visit.
Dr. Chapman accepts all major insurance carriers as well as Medicare.

Payment Options:
For your convenience, we accept all major credit cards for your co-payment.

Appointments:
Your first appointment involves a complete evaluation and examination. To help us, we ask that you bring the following to your first appointment:

• Written reports of all MRI,CT and X-Rays
• A referral from your Primary Care Physician if required by your insurance carrier
• Your insurance card and driver's license
• A list of your medications
• Your pharmacy profile
• Download, print and fill out your New Patient Paperwork
